Publication Metadata only Awareness of health sciences students about colorectal cancer risk factors(2019-05) Üstündağ, Hülya; Zengin, Neriman; Andsoy, Işıl Işık; GÜL, ASİYEObjective This descriptive study was conducted in order to determine the risk factors, symptoms and awareness of health sciences students with regards to colorectal cancer (CRC). Methods This study included 1,028 health science students from two different universities. Data for this study were collected via a questionnaire form that included questions about CRC risk factors, protective approaches and symptoms. Frequencies, percentages and averages of the data were assessed with Kruskal-Wallis and Mann-Whitney U tests. Results The most frequent risk factors of the students (mean age: 21.25 years) were smoking (90.5%), excessive alcohol consumption (87.4%), family history of cancer (84.2%) and obesity (82.6%), while the most common protective approaches were smoking cessation, avoiding alcohol, regular physical activity and a low-fat diet. There was a significant difference in the CRC awareness of students in terms of gender, the department and grade level at which they were studying, and having a family history of cancer. Conclusion Results of this study revealed that students had a sufficient awareness about risk factors and what they could do to diminish their risk, but their knowledge about CRC symptoms was less than expected. Bu çalışmanın amacı, ülkemizde hemşirelik alanında kuram ve modele dayalı yapılan lisansüstü tez çalışmalarının nitelik ve niceliğinin incelenmesidir. Yöntem: Çalışmanın örneklemini 2009-2019 yılları arasında Yüksek Öğretim Kurumu (YÖK) veri tabanına yüklenmiş, on-line tam metnine ulaşılabilen, hemşirelik kuram ve modellerine dayalı tasarlanmış 43 lisansüstü tez (11 yüksek lisans, 32 doktora) çalışması oluşturdu. Değerlendirme Fawcett ve Gigliotti’nin kuram/model kullanımında tanımladıkları beş basamak göz önünde bulundurularak yapıldı. Bu çalışmaya dahil edilen tüm araştırmaların etik kurul onayı mevcuttur. Bulgular: Çalışmaların çoğunlukla (%74,4) doktora tez çalışmalarından oluştuğu ve kullanılan modellerin başında sağlık inanç modeli (%25,0) ve Roy adaptasyon modelinin (%22,5) yer aldığı belirlendi. Çalışmaların %48,8’inin (n=21) randomize kontrollü olarak tasarlandığı ve sıklıkla Halk sağlığı hemşireliği (%25,6) alanında çalışıldığı görüldü. Doktora çalışmalarının %62,5’inde (n=20) kullanılan kuram veya modelin kavram, kuram ve deneysel yapısının belirtilmediği saptamrken; yüksek lisans çalışmalarında bu oran %63,6 (n=7) olarak belirlendi. Altı çalışmada (%14) aynı kuram veya model kullanılarak yapılmış benzer çalışmaların sonuçlarına yer verilmediği, sadece iki doktora çalışmasında ise (%4,7) kullanılan kuram veya modelin zayıf yönleri ve uygulamasında yaşanan zorlukların açıklandığı saptandı. Sonuç: İncelenen çalışmalarda, hemşirelik araştırmalarında kural ve model kullanımının sınırlı olduğu, kuram ve model kullanımı basamakları doğrultusunda bir standardizasyonun olmadığı belirlendi.Publication Metadata only The Importance and Prevention of Mastitis in Mothers(2019-06) GÜL, ASİYE; 112620Aim: This review emphasizes the incidence of mastitis in breastfeeding mothers and what needs to be done to prevent mastitis. Introduction: Mastitis, inflammation of the breast, refers to the inflammatory disease o f the breast. Mastitis is classified as lactating (puerperal), non-lactating and breast abscess. Lactational mastitis is the most common mastitis. Lactational mastitis is seen in 3-33%. It is more common in the first 12 weeks of breastfeeding. Most breast abscess develops as a complication of lactational mastitis. Milk stasis and infection are two main causes. Staphylococcus aureus is the most common causative agent for mastitis. Other organisms include streptococcus and S. epidermidis. Patients with recurrent breast abscess have an increased incidence of mixed flora, including anaerobic organisms. Breast pain is the primary symptom of mastitis. Other symptoms; vomiting with general flu-like symptoms such as high fever, weakness, muscle pain, sweating, and headache. In addition to physiological problems, such as depression, anxiety, and helplessness develop. This affects daily life activities and causes lifestyle changes. Providing the right breastfeeding milk flow is the main principle of treatment. Breastfeeding is not harmful to the baby, it is also the ideal way to drain the milk. Lack of education on postpartum breast care, reduction of local hygiene conditions, encouraging the mother to breastfeed, and correcting the newborns should be correctly positioned. Conclusion: The incidence of mastitis can be reduced by informing the mothers and changing their attitudes in a positive way.Publication Restricted Healthy Lifestyle Behaviors and Quality of Life After Sleeve Gastrectomy(Lippincott Williams & Wilkins, 2020) Üstündağ, Hülya; GÜL, ASİYE; Özkaya, Birgül; Zengin, NerimanThe aim of this study was to evaluate healthy lifestyle behaviors and the quality of life of obese individuals in 3-12 months after sleeve gastrectomy. The study is a descriptive design and was conducted at the obesity surgery outpatient clinic of a government hospital between May and December 2017. The study sample included 172 patients agreeing to participate. Data were gathered with a questionnaire composed of questions about sociodemographic features, the Health Promoting Lifestyle Profile II (HPLP II), and the WHO Quality of Life BREF-TR (WHOQOL-BREF-TR). The total HPLP II score was above average; participants had the highest and lowest scores for the subscales spiritual growth and physical activity, respectively. The total score for the WHOQOL-BREF-TR was above average. The highest and lowest scores were obtained for the subscales psychological health and social relations, respectively. There was a significant correlation between the scores for HPLP II and its subscales and the scores for WHOQOL-BREF-TR. Promotion of health and a healthy lifestyle need to be an integral part of health services provided for patients after sleeve gastrectomy.Publication Metadata only Validity and reliability study of Pieper Pressure Ulcer test in Tuskish Nurses(2019-06) GÜL, ASİYE; 112620Aim: The aim of the cross-sectional and descriptive study was to determine validity and reliability Pieper Pressure Ulcer Knowledge Test (PUKT) in Turkish Nurses. Methods: The sample consists of Turkish nurses who working in a training and research hospital in the European side of Istanbul. Data were collected using the PUKT. The PUKT was developed by Pieper in 1993 to evaluate pressure ulcer prevention, staging of pressure ulcers, and wound identification of nurses' knowledge. Lawrence et al. validated modified Pieper Pressure Ulcer Information Test in 2015. The test consists of 49 items, a correct answer was scored 1, and an incorrect response was scored 0, thus, the possible score range was from 0 to 49. Results: The language validity and cultural adaptation were performed of the PUKT. In the analysis, which was conducted according to expert opinions, the content validity index (CVI=0.918) were determined. The reliability of the scale was examined with time invariance and internal consistency. In the test-retest test, there was no statistically significant difference between the two measurement results (t = -1.875, p = 0.06), the correlation coefficient was 0.840 and the relationship between them was statistically significant. The Kuder Richardson 20 value, which was calculated for testing internal reliability, was found to be 0.735. The Kappa value was determined to be 0.646; thus, the test had good interrater reliability. Conclusion: it has been determined that Turkish form of PUKT is appropriate measurement tool in terms of language and content validity to Turkish nursing.Publication Metadata only Development of Fistula in a Patient With Crohn's Disease: a case report(2019-06) GÜL, ASİYE; 112620INTRODUCTION: A fistula is an abnormal connection between two epithelized surfaces. 15- 25% of the fistulas develop spontaneously and 75-85% develop after surgery. Spontaneous fistulas develop after radiotherapy, diverticulitis, appendicitis, peptic ulcer perforation, pancreatitis, intestinal tuberculosis, inflammatory bowel disease. Malnutrition, sepsis, shock, circulatory failure, corticosteroid treatment, difficulties in surgical anastomosis cause fistula formation. Fistula develops 20% -40% with Crohn's disease. 75% of intestinal fistulas open to the skin. It has psychological, physiological, sociological and economic effects in patients with fistula. Objective: In this study, it was emphasized the importance of nursing care of a patient with Crohn's disease, difficult management, and long-standing fistula. Case: Mr. ET who has Crohn's disease, is 27 years old, married, graduated from primary school, lived in the village outside of Istanbul. The patient admitted to the emergency department with severe abdominal pain at the time of his visit to his brother. The emergency operation performed due to Ileus and common small intestinal dilatation. Segmenter small intestine resection and ileostomy performed and the patient followed up in the intensive care unit. Because of intraabdominal abscess, a vacuum system is applied. Afterward, two fistulas developed in the abdominal region. The total parenteral and enteral food is used for weight loss. Antibiotic therapy initiated for infection. Three months later, the fistula healed and the patient discharged on his own request. Conclusion: Complicated nursing care and multidisciplinary care must be provide for the problems that may occur in the patient who is hospitalize for a long time after the fistula.Publication Metadata only The Most Commonly Determined Nursing Diagnoses According to NANDA-I Taxonomy and Difficulties Using the Nursing Process in COVID-19 Outbreak(Sage Publications Inc., 2022) Tosun, Hale; Tosun, Ayşe; Özkaya, Birgül Odul; GÜL, ASİYEThe study was planned to determine the most common nursing diagnoses according to NANDA International (NANDA-I) taxonomy and difficulties experienced in using of nursing process in COVID-19 outbreak. The sample of the descriptive cross-sectional study consisted of nurses cared for patients with COVID-19 (n = 114). Average age of nurses is 26.86 +/- 6.68. Commonly determined nursing diagnoses according to NANDA-I taxonomy in patients with COVID-19 were imbalanced nutrition (66.7%), impaired gas exchange (40.4%), insomnia (21.1%), acute confusion (31.6%), hopelessness (96.5%), difficulty playing caregiver (84.2%), anxiety (38.6%) willingness to strengthen religious bond (71.9%), risk for infection (64.9%), nausea (49.1%). Twenty-four-years old and younger, high school graduates, caring for intubated patients, and those who stated that they did not use nursing diagnosis had more difficulty in using nursing process (<0.05). The use of nursing diagnoses and process for patients with COVID-19 is extremely important in ensuring individual and qualified nursing care.Publication Open Access Cerrahi Girişim Uygulanan Hastalarda Ağrı ve Konfor İlişkisi(Oğuz KARABAY, 2022) Tosun, Hale; Özkaya, Birgül Ödül; Uz, Ferişte; GÜL, ASİYEAmaç: Çalışma cerrahi girişim uygulanan hastalarda ağrı ve konfor ilişkisini değerlendirmek amacıyla yapıldı. Materyal ve Metot: Bir eğitim ve araştırma hastanesinde cerrahi girişim uygulanan hastalar araştırma evrenini oluşturdu. Tanımlayıcı olan çalışmada örnekleme cerrahi girişim uygulanmış ve çalışmaya katılmayı kabul eden 132 hasta alındı. Veriler hasta tanım formu, Sayısal Derecelendirme Ölçeği ve Genel Konfor Ölçeği kullanılarak elde edildi. Bulgular: Örnekleme alınan hastaların yaş ortalamaları 42,64±17,27, %53’ü erkek, %58,3’ü evlidir. Katılımcıların %72’sinin herhangi bir kronik hastalığı bulunmamaktadır. Daha önce cerrahi bir girişim uygulananların oranı %16,7’dir. Çalışmada en fazla uygulanan cerrahi işlemin laparoskopik kolesistektomi (%19,7) ve laparoskopik apendektomi (%16,7) olduğu belirlendi. Genel anestezi %80,3’üne uygulandı. Hastaların ağrı puan ortalaması 3,68±1,37, Genel Konfor Ölçeği toplam puanı 136,46±11,20 olarak belirlendi. Ameliyat sonrası hastaların %59,1’inin orta derecede ağrısı bulunmaktaydı. Ağrı ile Genel Konfor Ölçeği toplam puan ortalaması arasında negatif yönlü çok zayıf bir ilişki belirlendi (r=-0,175; p=0,045). Sonuç: Ameliyat sonrası ağrı rahatsızlık yaratan bir durumdur. Ağrı ve konfor düzeyinin belirlenmesi, bakımın kalitesini ve hasta memnuniyetini arttırmak için önemlidir.Publication Metadata only "Sleep Quality and Depression Level in Nurses in COVID-19 Pandemic"(Sage Publications Inc., 2022) Tosun, Ayşe; Tosun, Hale; Özkaya, Birgül Ödül; Erdoğan, Zeynep; GÜL, ASİYEThe COVID-19 spread rapidly all over the world and pandemic process has negatively affected nurses. Sleep disorders and depression are among these negative effects. Managers' awareness of problems experienced by nurses and taking precautions will increase employee and patient satisfaction and provide quality patient care. The study was conducted in descriptive, cross-sectional and correlational design to explain nurses' sleep quality and depression levels and relationship between them during the COVID-19 pandemic with 142 nurses who completed Personel Information Form, Pittsburgh Sleep Quality Index (PSQI) and Beck Depression Index (BDI). Sleep quality of nurses (64.8%) was poor, 33.1% had depression, and high school education, having a chronic illness and PSQI score increased the risk of depression level of nurses. Results highlight COVID-19 pandemic affected nurses' sleep quality and depression levels. All managers should be pioneers in providing psychological support to nurses and preparing and implementing a program for prevention of insomnia and depression.
